SNOW SLIDE WAIVER

Snow Tubing Waiver Terms & Conditions

• I, being over eighteen (18) years of age, or if Participant is under eighteen (18) years of age, I, Parent or Guardian of an underage Participant, agree to release Main Street of Perry, its agents, volunteers, and employees from any liability to anyone who may be injured or damaged in any way as a direct or indirect result of contact with Snow Tubing and/or the product delivered by Emergency Ice during and after the Snow Tubing event. Further, I also agree to defend and hold Main Street of Perry, its agents, servants and employees harmless from any claim(s) made during or after the event directly or indirectly as a result of the use of Snow Tubing and/or the Emergency Ice product(s), except claims resulting from the negligence or willful misconduct of Main Street of Perry.
   

• The Participant acknowledges and agrees that the Participant will not be permitted to participate in the Snow Tubing activity unless the Participant, or the parent or guardian of an underaged Participant, signs this waiver.
   

• The Participant fully understands the risks and claims that the Participant is waiving to participate in the Snow Tubing activity.
   

• The Participant acknowledges and agrees that the Participant is forever prevented from suing or otherwise claiming against the Activity Provider and/or Main Street of Perry for any property loss or personal injury that the Participant may sustain while participating in or preparing for the Snow Tubing activity.
   

• Main Street of Perry reserves the right to exclude any Participants for unruly and/or unsafe behavior related to the Snow Tubing activity and/or event.
   

• This Waiver will be governed by the State of Oklahoma.

I have read and understand the above terms and agree to be bound by them.
